Instructions:
1) Select your Web Form Page, from your hard drive, by using the 'browse' button below.
2) Your page must have valid FORM start and end tags and at least one form input field.
3) Only one FORM statement is allowed per page.
4) 150 maximum form input fields are allowed.
5) Only one PASSWORD and one FILE field are allowed per form.
6) Use meaningful/descriptive INPUT field names. These names will label the data you receive from your clients.
7) If possible, remove any javascript. It could confilct with our javascript edits.
Note:
Be sure to save your original Web Form Page file in order to apply modifications.
